Can the gut microbiome signal diabetes risk years ahead?

Most of what we know about the gut microbiome and type 2 diabetes comes from snapshots taken after people are already diagnosed, which makes it hard to tell whether microbial differences are a cause, a consequence, or a side effect of medication. Could the microbiome, measured years earlier, hint at who later develops the disease? A large Swedish study set out to look
Researchers followed 4,685 older adults (average age 74) who were free of diabetes and not taking diabetes medication at the outset. They profiled each person’s gut bacteria in detail using whole-genome sequencing, then tracked who developed type 2 diabetes over roughly the next five years; 383 people (about 8%) did. As an observational study, it can reveal associations but cannot prove that any microbe causes diabetesNine bacterial species were consistently linked to future risk – six with higher risk (including Desulfovibrio piger, Ruminococcus gnavus and, unexpectedly, Akkermansia muciniphila, a microbe often regarded as beneficial) and three with lower risk (such as Coprococcus catus).
The individual effects were modest. Notably, the higher-risk signal for A. muciniphila was strongest in people eating the least fibre (under about 21 g a day), hinting that diet may shape whether a given microbe helps or hinders. Overall microbial diversity, often assumed to matter, showed no clear association.
Because the microbiome was measured well before diagnosis, the findings strengthen the idea that gut bacteria are part of the diabetes story rather than simply a result of it – though they remain associations, not proof. The group was older and Swedish, so the results may not extend to other populations, and only one species matched an earlier large study. Even so, the fibre-dependent twist is a useful reminder that microbial “good” and “bad” labels are rarely absolute – and a line of research worth watching.
